Understanding Knee Pain

Knee pain is common and can result from arthritis, overuse, injury, muscle weakness, or problems involving the bones, cartilage, ligaments, tendons, or other structures around the joint.

The knee is a complex joint that helps support body weight while allowing walking, standing, climbing stairs, bending, and many other daily activities.

Because the knee is used so often, pain can significantly affect mobility, independence, work, exercise, and quality of life.

What Does Knee Pain Feel Like?

Knee pain can vary depending on the cause.

Symptoms may include:

  • Aching
  • Sharp or stabbing pain
  • Stiffness
  • Swelling
  • Tenderness
  • Grinding or crackling sensations
  • Pain with walking
  • Pain when climbing stairs
  • Pain when standing from a chair
  • Pain with bending or squatting
  • Difficulty fully bending or straightening the knee
  • A feeling that the knee may give way

Some people experience pain only with activity, while others may have discomfort at rest or at night.

What Can Cause Knee Pain?

Knee pain has many possible causes.

Common causes include:

  • Osteoarthritis
  • Muscle weakness
  • Overuse
  • Tendon irritation
  • Bursitis
  • Meniscus injury
  • Ligament injury
  • Patellofemoral pain
  • Previous knee injury
  • Repetitive physical activity
  • Changes in walking or body mechanics

The location, timing, and type of pain can provide clues about what may be contributing.

Knee Osteoarthritis

Osteoarthritis is one of the most common causes of chronic knee pain.

It may cause:

  • Aching
  • Stiffness
  • Reduced range of motion
  • Swelling
  • Pain with walking
  • Difficulty climbing stairs
  • Pain when standing from a seated position

Symptoms may gradually worsen over time but can also fluctuate.

Osteoarthritis does not automatically mean someone will eventually need surgery. Many people manage symptoms for years with exercise, strengthening, weight management when appropriate, physical therapy, and other conservative treatments.

Patellofemoral Pain

Patellofemoral pain occurs around or behind the kneecap.

It may become worse with:

  • Climbing stairs
  • Squatting
  • Running
  • Sitting for long periods
  • Repeated bending of the knee

Muscle strength, movement patterns, training volume, and biomechanics may all contribute.

Meniscus Injuries

The menisci are pieces of cartilage that help absorb force within the knee.

Meniscus injuries may cause:

  • Joint-line pain
  • Swelling
  • Clicking
  • Catching
  • Difficulty bending the knee

Some meniscal changes develop gradually with age, while others occur after twisting injuries.

Not every meniscus problem requires surgery.

Ligament Injuries

Ligaments help stabilize the knee.

Injuries may occur during sports, falls, twisting movements, or trauma.

Symptoms may include:

  • Sudden pain
  • Swelling
  • Instability
  • Difficulty walking
  • A feeling that the knee is giving way

Significant injuries should be medically evaluated.

Tendon and Bursal Pain

Tendons connect muscles to bones, while bursae are small fluid-filled structures that help reduce friction.

Irritation of these tissues may cause pain around the knee, particularly with repeated activity.

Examples include:

  • Patellar tendinopathy
  • Quadriceps tendinopathy
  • Pes anserine bursitis

How Is Knee Pain Evaluated?

A healthcare professional may ask about:

  • When the pain started
  • Where the pain is located
  • Whether there was an injury
  • Activities that make the pain worse
  • Swelling
  • Locking or catching
  • Instability
  • Previous knee injuries or surgeries
  • Difficulty walking or climbing stairs

A physical examination may evaluate:

  • Range of motion
  • Strength
  • Swelling
  • Tenderness
  • Stability
  • Walking
  • Alignment
  • Function

Is Imaging Always Necessary?

Not every person with knee pain needs imaging.

X-rays may be useful when osteoarthritis, fracture, or other bony changes are suspected.

MRI may sometimes be considered when there is concern for:

  • Meniscus injury
  • Ligament injury
  • Cartilage injury
  • Other soft-tissue problems

Imaging results should be interpreted together with symptoms and physical examination findings.

What Can Help Knee Pain?

Treatment depends on the cause, but conservative care often includes:

  • Exercise
  • Strengthening
  • Physical therapy
  • Activity modification
  • Weight management when appropriate
  • Heat or cold
  • Assistive devices
  • Non-opioid medication when appropriate

Exercise and Movement

Appropriate exercise can help improve:

  • Strength
  • Stability
  • Mobility
  • Balance
  • Walking
  • Confidence with movement

Helpful activities may include:

  • Walking
  • Cycling
  • Water exercise
  • Strength training
  • Range-of-motion exercises
  • Low-impact aerobic exercise

The goal is usually to remain active while adjusting activities that significantly aggravate symptoms.

Strengthening

Strengthening the muscles around the knee can help support the joint.

Important muscle groups include:

  • Quadriceps
  • Hamstrings
  • Hip muscles
  • Gluteal muscles

Improving strength can reduce stress on the knee and make everyday activities easier.

Physical Therapy

Physical therapy may help with:

  • Strength
  • Flexibility
  • Balance
  • Walking
  • Stair climbing
  • Movement mechanics
  • Return to activity

A physical therapist can also help identify movement patterns that may contribute to pain.

Weight Management

For people who are overweight or obese, excess body weight can increase stress on the knees.

Weight reduction may help some people with knee osteoarthritis improve:

  • Pain
  • Walking
  • Mobility
  • Function

Weight management should be considered as one part of a broader treatment plan rather than the sole explanation for knee pain.

Heat and Cold

Heat

Heat may help:

  • Reduce stiffness
  • Relax surrounding muscles
  • Make movement more comfortable

Cold

Cold may help temporarily reduce:

  • Pain
  • Swelling
  • Discomfort after activity

Protect the skin when using either heat or cold.

Braces and Assistive Devices

Some people may benefit from:

  • Knee braces
  • Canes
  • Walkers
  • Supportive footwear
  • Other assistive devices

These tools can help improve safety or reduce stress on the knee when appropriately selected.

What About Medication?

Medication may be used as one part of a broader treatment plan.

Depending on the cause and your medical history, a healthcare professional may discuss:

  • Acetaminophen
  • Topical NSAIDs
  • Oral NSAIDs
  • Other topical pain-relief products
  • Other non-opioid medications

NSAIDs may not be appropriate for everyone.

Additional caution may be needed for people with:

  • Kidney disease
  • Heart disease
  • High blood pressure
  • History of stomach ulcers
  • Gastrointestinal bleeding
  • Certain medication interactions

Discuss benefits and risks with your healthcare professional.

Joint Injections

For some people with persistent knee pain, a clinician may discuss joint injections.

These may sometimes be used to provide temporary symptom relief.

Benefits vary from person to person.

Ask about:

  • Expected benefit
  • Duration of relief
  • Potential risks
  • Alternatives
  • Whether injections are appropriate for your specific condition

What About Opioids?

Opioids are generally not a first-line treatment for chronic knee pain.

Treatment usually emphasizes:

  • Exercise
  • Strengthening
  • Physical therapy
  • Activity modification
  • Weight management when appropriate
  • Non-opioid medication
  • Other evidence-based treatments

Opioids can carry risks including:

  • Sedation
  • Constipation
  • Falls
  • Physical dependence
  • Opioid use disorder
  • Respiratory depression
  • Overdose

When Is Surgery Considered?

Surgery may be considered when knee problems significantly limit function and appropriate conservative treatment has not provided enough relief.

Possible procedures depend on the underlying condition.

For severe knee osteoarthritis, joint replacement may be considered when pain and disability remain substantial despite nonsurgical treatment.

The decision should be based on:

  • Symptoms
  • Function
  • Imaging
  • Overall health
  • Treatment history
  • Personal goals

Focus on Function

Pain intensity is only one part of knee health.

Consider whether treatment helps you:

  • Walk farther
  • Climb stairs
  • Stand from a chair
  • Exercise
  • Sleep better
  • Return to work
  • Participate in hobbies
  • Care for yourself independently

Functional improvement can be a meaningful sign that treatment is working.

Questions to Ask Your Healthcare Professional

Consider asking:

  • What may be causing my knee pain?
  • Do I need imaging?
  • Would physical therapy help?
  • What exercises are safe for me?
  • Would strengthening help?
  • Would weight management improve my symptoms?
  • Are topical medications appropriate?
  • Would a brace or cane help?
  • Should I consider an injection?
  • When should I see an orthopedic specialist?

When Should You Seek Medical Attention?

Contact a healthcare professional if knee pain:

  • Persists or worsens
  • Significantly affects walking
  • Causes repeated giving-way
  • Is associated with persistent swelling
  • Limits daily activities
  • Does not improve with reasonable self-care

Seek prompt or urgent evaluation if:

  • You cannot bear weight after an injury
  • The knee becomes severely swollen
  • The joint appears deformed
  • The knee becomes suddenly hot, red, and very painful
  • Knee pain occurs with fever
  • The knee locks and cannot move normally
  • Significant weakness or numbness develops

A hot, swollen joint with fever may indicate infection and requires prompt medical attention.

Living Well With Knee Pain

Knee pain does not always mean you should stop moving.

In many cases, appropriate exercise and strengthening are important parts of treatment.

Progress may involve:

  • Gradually increasing activity
  • Improving muscle strength
  • Adjusting aggravating activities
  • Managing body weight when appropriate
  • Improving balance
  • Using assistive devices when needed
  • Setting realistic functional goals

Instead of asking only:

“How much does my knee hurt?”

also consider:

“Am I walking better, moving more confidently, and doing more of the activities that matter to me?”

Those changes can provide a more complete picture of progress.
