Understanding Neuropathic Pain and Peripheral Neuropathy

Neuropathic pain is pain caused by damage or disease affecting the nerves or nervous system. It often feels different from the aching or soreness associated with muscles and joints.

Neuropathic pain can occur when nerves are injured, compressed, inflamed, or affected by certain medical conditions.

What Is Peripheral Neuropathy?

  • Managing autoimmune or metabolic conditions

In some cases, nerve damage cannot be fully reversed, but identifying the cause may help slow progression.

  • Topical lidocaine
  • Capsaicin

These medications work differently from traditional pain relievers because neuropathic pain involves abnormal nerve signaling.

Not every medication is appropriate for every patient.

Possible side effects can include:

  • Dizziness
  • Drowsiness
  • Swelling
  • Nausea
  • Changes in blood pressure
  • Increased fall risk

  • Mindfulness
  • Meditation
  • Cognitive behavioral therapy
  • Counseling
  • Support groups
  • Music or other calming activities

These approaches can complement medical and rehabilitative treatment.

What About Opioids?

Opioids are generally not considered a first-line treatment for most chronic neuropathic pain conditions.

Other treatments are usually considered first because neuropathic pain often responds better to therapies that target abnormal nerve signaling.

Long-term opioid treatment may also carry risks such as:

  • Sedation
  • Constipation
  • Falls
  • Physical dependence
  • Opioid use disorder
  • Respiratory depression
  • Overdose

If opioids are considered, the potential benefits and risks should be carefully evaluated by a healthcare professional.

Preventing Falls and Injuries

Peripheral neuropathy can affect balance and sensation.

Consider discussing fall prevention with your healthcare team if you experience:

  • Frequent stumbling
  • Difficulty walking in the dark
  • Loss of balance
  • Weakness
  • Repeated falls

Questions to Ask Your Healthcare Professional

Consider asking:

  • What may be causing my nerve pain?
  • Do I need testing for peripheral neuropathy?
  • Could diabetes or another medical condition be contributing?
  • Are any of my medications associated with neuropathy?
  • Which treatments are most appropriate for my symptoms?
  • Would physical therapy help?
  • How can I protect areas where I have numbness?
  • What medication side effects should I watch for?
  • What can I do to reduce my fall risk?
  • When should I see a neurologist or other specialist?

When Should You Seek Medical Attention?

Contact a healthcare professional if you develop:

  • New or worsening numbness
  • Persistent burning or tingling
  • Progressive weakness
  • Balance problems
  • Frequent falls
  • New wounds or sores that are difficult to feel
  • Symptoms affecting daily activities

Seek urgent medical attention for symptoms such as:

  • Sudden difficulty speaking
  • New loss of bladder or bowel control
  • Rapidly worsening weakness
  • Severe back pain with new leg weakness or numbness around the groin

“What can I do today that helps me stay safe, active, and independent?”

That perspective can help make pain management more focused on meaningful daily life.
